Free Mental Health Assessment
Better understand what you are experiencing and whether talking with a mental health professional may help.
No online test can diagnose you with a condition, but it can help you understand your symptoms and whether to seek care.
About this free mental health assessment
This mental health test is free and private, and it is not a diagnosis. No online test can diagnose you with a condition, but it can help you understand your symptoms and whether to seek care. Your results appear on screen as soon as you finish the last step.
Before your results are shown, you enter your first name, last name and email so MindVibe can follow up. A phone number is optional. Your responses and scores will be submitted securely to MindVibe.
The screeners this mental health assessment includes
Each screener is a short questionnaire about one concern. Choose the ones that match what you are feeling.
Depression (PHQ-9)
Depression screening: 9 questions about the past 2 weeks.
Anxiety (GAD-7)
Anxiety screening: 7 questions about the past 2 weeks.
ADHD (ASRS-5)
Adult ADHD screening: 6 questions about the past 6 months.
Insomnia (ISI)
Sleep assessment: 7 questions about the past 2 weeks.
Substance (CAGE-AID)
Substance screening: 4 questions about the past year.
Trauma (Brief Trauma Screener)
PTSD screening: 5 questions about the past month.
Safety (C-SSRS)
Safety screening: 6 questions about the past month.
Choosing Not Sure runs the Depression, Anxiety and ADHD screeners together, about 9 minutes in all. If you choose Self-Harm, or an answer suggests thoughts of self-harm, the Safety screener runs and the page ends with crisis support rather than a results screen.
How it works
- Choose one or more concerns that match what brings you here.
- Answer each question one at a time. You can go back to change an answer. A single screener takes about 2 to 4 minutes.
- Enter your name and email, agree to the consent statement, and view your results. Each result shows your score, the range it falls in and a short note.
- Decide what to do next. These results are screening guidance, not a diagnosis. A licensed professional can help you understand them in context. You can book an appointment from the results page or retake the assessment.
